Expert assessment and advice

To ensure that the correct chronic medication has been prescribed, our panel of pharmacological and medical advisors will assess the medication in accordance with clinical guidelines and may suggest alternatives, where required.

We also provide advice and information regarding chronic conditions.

With our medical experts and technology, we have made our chronic authorisation process highly efficient, so we won't keep you waiting on an authorisation for the medication you require.

For medication authorisations

    Download the MRM Programme application form.

    Complete the member and patient information.
  • 3
    Ask your treating doctor to complete the medication practitioner's information and clinical examination.
  • 4
    Both the member/patient and the treating doctor are required to sign the completed form.
  • 5
    Return the completed form to the Medicine Risk Management Programme on the details provided on the application form.

Chronic medication advance supply request form

Please complete this application form if you have any circumstances, such as plans to travel outside of the country for an extended period, and you need to request an advance supply of medication from your doctor/pharmacy.
