The Integrated Care Programme (formerly called the Disease Risk Management Programme) identifies beneficiaries who have respiratory, cardiovascular system, diabetic, renal (kidney) and mental health conditions.

Upon identifying the beneficiary, they are automatically enrolled on the programme to assist with management of their condition(s). Once enrolled, you will receive an individual treatment plan to manage your condition(s)

Care coach

This programme offers you direct access to a specialised care coach. Your care coach will work closely with you and your doctor to ensure that your care is co-ordinated and you receive maximum value from the Society.

Care Plan
  • Care plans provide patients with:
  • •  authorised chronic medication
  • •  authorised consultations, if required
  • •  authorised tests and investigations, if required
  • •  education material regarding your condition(s).

  • Integrated care is uniquely tailored to the specific health needs and profile of each participating member. You will be taken on a personalised journey towards better health. Having a chronic condition does not stop you from living your best life. Read more to find out how you can do that.

  • We automatically identify members who will benefit from enrolling on the programme. This usually includes members registered for a relevant chronic condition or who are on chronic medication. Members who will benefit from the programme are then requested to complete an enrolment form and return it to us.

Palliative care programme leaflet

BP Medical Aid Society has partnered with Momentum Health Solutions, the Society's healthcare administrator, to provide our valued members with access to a Palliative Care Programme. Palliative care is specialised medical care for people living with a serious illness and is focused on providing relief from the symptoms and stress of their condition(s).
